Cholecystocolonic fistula presenting as acute pancreatitis

Abstract
Cholecystocolonic fistula, which is defined by an abnormal communication between the gallbladder and colon, is a rare complication of cholelithiasis. Cholecystocolonic fistula represent the second most common form of spontaneous biliary enteric fistula and its incidence is from 10% to 20% of all biliary enteric fistulas. There are usually no specific signs and symptoms of cholecystocolonic fistxila, so the presence of the fistula is diagnosed preoperatively under 50% of cases.
We report the case of a 63-year-old male with cholecystocolonic fistula between the gallbladder and the hepatic flexure of the colon. The symptom was severe epigastric pain. Serum amylase and lipase were elevated markedly. Abdominal computed tomography and endoscopic retrograde cholangi opancreatography demonstrated common bile duct stone and cholecystocolonic fistula. The patient was treated with an open cholecystectomy, a resection of the fistula and a primary repair of transverse colon.
